This letter constitutes our response to the item of noncompliance identified in the referenced inspection report.
It is our opinion that nothing in the report or in this response is proprietary in nature.
Our response to the item of noncompliance listed in Appendix "A" Notice of Violation is as follows:
"... Shim thickness shall be selected so that the total weld thickness being radiographed under the penetrameter is the same as the total weld thickness..."

Corrective Action Taken and Results Achieved Records for the two welds identified, CY-606 and WR-4, have been reviewed.
WR-4 was removed in accordance with engineering change request (ECR) number 1149 and replaced with weld K 2031.
This ECR was initiated on September 3, 1980, to reroute the pipe and components and to improve seismic support.
CY-606 was initially radiographed on July 15, 1976, as noted in IE Inspection Report No. 50-358/80-09. 'Three repairs were made to the weld and final radiographic acceptance achieved on August 11, 1976.
The final radiographic report showed four circumferential views and was accepted by all reviewers and the 81"10190685 811009 PM ADOCK 05000358 e3 PDR SEP 2 9 1981

This implies that the film did have properly spaced penetrameters.
Two of the original four views were not readily available and were rcradiographed, each with a properly shimmed penetrameter.
(b)
Corrective Action Taken to Avoid Further Concompliance The Quality Confirmation Program (QCP) committed to the NRC in letter QA-1476 dated August 21, 1981, details the action to be taken to resolve the acceptability of radiographs using unshimmed or under-shimmed penetrameters.
For all ongoing work, NDE field personnel have been instructed to shim penetra-meters in accordance with code requirements.

Date When Full Compliance Will Be Achieved Full compliance for ongoing radiography has been achieved.
Verification of the acceptability of radiographs using under-shimmed penetrameters will be achieved in accordance with the QCP.
"...When the source is placed on the axis of the object and the complete circumference is radiographed with a single exposure, at least three uniformly spaced penetrameters shall be employed."

Corrective Action Taken and Results Achieved As stated above, the originally accepted radiograph of CY-606 showed four views.
Proper spacing of the penetrameter 120 apart may result in one view not having a penetrameter.
Reradiographc of these two views as noted above included properly shimmed penetrameters and resulted in a circumferential radiograph utilizing three (3) penetrameters which were not equally spaced due to the radiography being performed at different times.

Corrective Action Taken 'o Avoid Further Noncompliance Because CY-606 is not readily accessible and is full of water, the review of CY-606 has not been completed.

An additional thirty days is required to complete this review.
