It has been discovered that eight flywheel bearing pedestal holddown bolt lockwashers on the Browns Ferry Nuclear Plant unit 1 low pressure coolant injection motor generator sets were broken. This condition was caused by improper heat treatment of the lockwashers by the vendor.

This problem, which is reportable under 10 CFR 21, was reported to me on October 7, 1981.

Unit 1 Low Pressure Coolant Injection Motor Generator Set.

Flywheel Bearing Pedestal Holddown Bolt Inckwashers The motor generator sets installed'on unit I have i generic problem witit breakage of the vwidor supplied flywheel bearing pedestal holddown bolt lockwashers.

This problem was also identified on the motor generator sets installed on unit 3.

Each motor generater set has four bearing pedestal holddown bolts which,have one lockwasher per bolt. An inspection conducte.d on August 26,198I found two of the unit 1 low pressure coolant

~' injectioi sotor generitor' sets installed and 'op'erating/' The other two '

motor generator sets on unit 1 were in the. process of. being. installed and were not operational. Of the two operational motor generator sets, one broken lockwasher per motor generator set was found.

Of the two nonoperational motor generator sets,. two broken lockwashers on one motor generator set and four broken lockwashers on the other motor generator set were found. A total of eight broken lockwashers were found on unit I which was exactly half of the total lockwashers.

i Pieces of these broken lockwashers were field checked with a hardness-l tester. Extremely wide hardness variations were found. Hardness values ranged from side to side on some of the same pieces by as much as 20 points on the Rockwell C scale.

It is apparent that the vendor supplied lockwashers did not have the proper heat. treatment _..

All of the motor generator sets for the low pressure coolant injection

. modifications.on.all three units were received.at the,same time.

Since,,

~

units 1 and. 3 were found to have ' problems with their lockwashers, it' is very likely that the unit 2 motor generator sets will have similar problems.

The problems on unit 1 will be resolved by replacing all lockwashers. The unit 3 problems were resolved by replacing the lockwashers.

Unit 2 motor generator sets have not been installed.
