o Appendix A NOTICE OF VIOLATION Victoreen, Inco rpo ra ted License No. 34-00486-94 License No. 34-00486-05 License No. 34-00486-09 As a result of the inspection conducted on September 22, 1981, and in ac-cordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),
the following violation was identified:
License Condition No. 17 of License ho. 34-00486-04 and License No. 34-00486-09, and License Condition so. 15 of License No. 34-00486-05 requires that licensed Material be pcasessed an? used in accordance with stacements, representations and procedures contained in referenced ap-plications aad letters.
Item 5 of application dated August 10, 1977, and Item 7 of applications dated December 28, and September 28, 1979, name Mr. John C. Miller as the Radiation Protection Officer.
Contrary to the above, the individual designated as your Radiation Protec-tion Officer was changed anc you failed to amend your licenses to reflect this change.
Thi is a Severity Level V violation (Supplement VII).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within t'irty days of the date of this Notice a written statement a
or explanation in reply, including for each item of noncompliance:
(1) cor-rective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or a ffinna tion.
Consideration may be given to extending your response time for good cause shown.
