Dear Mr. Streeter:

This is in response to your Notice of Violation and Investigation Report Number 50-461/81-18.

Illinals Power Company s responses to the two items of noncompliance cited are as follows:

1.

Instructions, Procedures and Drawings This Notice of Violation states, in part:

" Contrary to the above the Zack Company did not prepare a trend analysis for the Clinton site for January 1981, even though thirteen Nonconformance Reports (NCRs) were prepared at the Clinton site. during January 1981, for nonconforming (sic) conditions pertaining to drcwing M14-1108-6."

A.

Corrective action taken and th,e results achieved:

The Zack procedure, PQC-20, Rev. 0, " Trend Analysis" was a new procedure approved during Decenber, 1980.

Due to a misunderstanding by responsible supervisory personnel of Zack, the procedure was not implemented during January, 1981.

The procedure was implemented in February, 1981 and trend analysis has been performed on a-monthly basis since then.

B.

Corrective action taken to avoid further noncompliance:

In order to prevent future noncompliances of this nature the cognizant QA Engineer has received additional training in the implementation of the trend analysis procedure.

C.

Date when full compliance will be achieved:

Zack is now in full compliance with regulatory and procedural requirements.

Nonconforming Materials, Parts, or Components The Notice ot~ Violation states, in part:

" Contrary to the above, Zack Nonconformance Report No.

ZC-CB-325, prepared on December 12, 1980; was closed by the Zack Company on March 13, 1981, and was never forwarded to Baldwin Associates for review by the consulting engineers (Sargent & Lundy).

Additioaally, the nonconforming items were never individually tagged to identify the duct pieces as being nonconforming.

The NCR pertained to the qualification of Zack welders performing butt welds."

A.

Corrective action taken and the results achieved:

Zack nonconformance EC-CB-325 was not, as stated in the Notice of Violation, closed on March 13, 1981.

Rather, March 13, 1981 is the date the Zack Project Manager signed the block on the form for " Corrective Actions Taken" and forwarded the nonconformance to the Zack QC l

Manager for further processing.

When the above stated concern was brought to our attention by the inspector, the nonconformance report was located and further processing continued.

Subsequently, revisions to the NCR were made on June 8 and June 15, 1981 in order to properly identify materials.

The NCR was thereafter forwarded to Baldwin Asscciates where it was transferred to BA Nonconformanc report no. 5340'for Sargent & Lundy review and processing.

B.

Cerrective Action taken to avoid further noncompliance:

To prevent future noncompliances of this nature, responsible Zack personnel have and will continue to receive periodic training in the issuing and processing of nonconformance reports.

In addition, Zack Company is developing a computer listing of project NCRs which will be used as a management tool to assist in trend analysis and tracking of NCR status.

Although this program is currently under development, initial printouts have been reviewed and indicate that it will be beneficial in preventing problems similar to those addressed in this report.

C.

Date when full compliance will be achieved:

Zack is now in full compliance with regulatory and procedural requirements.
