The certificate will expire on March 31, 1982. Because the certificate references consist of an application and nany supplements, we request that you submit a consolidated application, with drawings, incorporating all previous submittals. The consolidated application need not iriclude your QA program but should continue to include quality control rwuirements and operating procedures.
The consolidated application (8 copies) should be submitted at least thirty (30) days prior to the expiration date of the certificate.
In addition, the renewal fee pscribed in 10 CFR 5170.31 should be included with your request.
