Contract No. NRC-05-80-251, Task Order No. 16 Pursuant to the pertinent provisions of this contract, I hereby authorize the expenditure of funds in the amount of $30,000.00 to provide documentation of the close out of Inspection and Enforcement Bulletin 80-08, Containment Liner Penetration Welds, outlined in the enclosed Task Order No.16.
If you believe that the total ceiling price is inadequate for the purposes of this Task Order, you must so notify me within ten (10) business days after its receipt. This notification shall contain your estimate of the required total ceilino cost.
Within ten (10) business days after receipt of such notification, tne Contracting Officer shall either ratify the total ceiling ccst or adopt the proposed revised estimate or some combination of the two and revise or confirm the Task Order accordingly.
This letter, executed on behalf of-the Commission, is forwarded to you in quadruplicate.
Please acknowledge receipt on three (3) topies hereon and return them to me as soon as possible.
The forth copy is for your retention.

Contract No. NRC-05-80-251 Task Order No. 16 Containment Liner Penetration Welds 1.
Scope of Work Provide documentation of the close out of IE Bulletin 80-08, Containment Liner Penetration Welds, as outlined in the statement of work, below.

Background===
3 When significant safety issues are identified by NRC, IE will frequently issue a bulletin which identifies the concern, actions to be taken by the affected utilities, and the information to be reported to NRC in a written response.
Typically, these reports are screened by the cognizant IE headquarters staff engineer.
Additionally, IE Manual 92703 provides for review by the regional inspectors and follow-up to insure that cor-rective actions committed to in the response have been carried out properly.
These review and follow-up actions are included in the inspection reports covering the period of time during which they took place.

Statement of Work a.
Review the bulletin and background documents to determine the safety significance of the concern, the salient actions required by the bulletin, and the plants requiring action and/or response.
b.
Review the uti,ity responses to the bulletin to determine compliance with the actions required by the bulletin, commitments for future actiors, results of inspections and/or tasts performed, components replaced and/or repaired, and manpower expenditures.
c.
Upon completio,i of a and b, above, the IE headquarters contact shall be provided (1) a summary of utilities not responding, either partially or fully, to the concerns raised by the bulletin, ar. ? (2) a list identifying those utilities proposing alternative re,olutions, along with a preliminary evaluation.of these alternatives.
d.
Review the inspection reports addressing the bulletin to determine the inspectors' evaluation of the utility responses and the status and/or completion of actions committed to in the responses.
e.
Upon completion of d, above, the IE headquarters contact shall be provided a summary of areas identified by the Inspection reports as being unacceptable.
f.
Based on the results of c and e, above, tentative conclusions and reconmendations should be provided the IE headquarters contact.
These conclusions and recommendations should cover (1) any identified inadequacies in the definition of the problem, the licensees responses and/or the inspection results; (2) any needed long term continuing actions (e.g., new inspection modules, revised standard review plan, etc..); and (3) the method of assuring that problems are resolved on those plants that were sent the bulletin for information, rather than action.

Prepare a report providing the documentation of the close out of the bulletin which contains the following:
(1) An introduction which identifies the safety significance of the issue at i includes the relationship to other bulletins, circulars, or infonnation notices and actions in the other offices of tiRC.
(2) Background which identifies the history of the events which prompted the bulletin.
(3) Salient actions required by the bulletin including identifi-cation of the plants requiring action and/or response.
(4) Overall summary of the responses identifying the types of actions taken by the utilities, results of inspections and/
or tests, components replaced and/or repaired, manpower expenditures, and regional inspector comments.
(5)
Identification of areas of concern uncovered or remaining along with recommendations for their resolution or for further actions to lead to resolution.
(6) Recommendations, if appropriate, regarding long term actions.
Also any special actions recommended for plants that were sent the bulletin for information, rather than action, should be identified.
(7) An appendix which provides, for each applicable plant:
(a)
The date(s) of the response (s) along with a summary of the actions taken in response to the salient features identified above, including commitments made.
(b) A summary of the results of inspections and/or tests made tc comply with the above conmitments.
(c)
Identification of the components repaired and/or replaced.
(d)
Identification of the inspection report (s) addressing the acceptability of the response (s) along with a summary of any salient comments.
(e)
Identification of the inspection report (s) addressing the status and/or completion of actions committed to in the response (s) along with a summary of any salient comments.
(f) Manpower expenditures reported by the utilities for the bulletin including that for any necessary corrective action.
