Subject:

SUPPLY SYSTEM NUCLEAR PROJECT NO. 2 10CFR50.55(e) REPORTABLE CONDITION #159 4

rc DEFECTIVE GE TYPE CR2940 CONTROL SWITCHES

Reference:

Letter #G02-81-0172, dated July 9,1981.

The referenced letter relates a reportable deficiency of a generic nature involving GE type CR2940 control switches, found to have contact blocks which came loose during shipment. This was officially reported by GE under 10CFR Part 21 on May 28,1981.

The referenced letter also states that GE-NED had completed inspection of all CR2940 switches within their scope on WNP-2. Corrective action has been determined for switches outside GE-NED's work scope on WNP-2 and is included herewith.

This is our final report on this condition.

Very truly yours, Ic RGM/SLN/kh P ogram director, WNP-2

FINAL REPORT WASHINGTON PUBLIC POWER SUPPLY SYSTEM DOCKET N0. 50-397 LICENSE NO. CPPR-93

.10CFR50.55(e) #159 REPORTABLE. DEFICIENCY ON GE TYPE CR2940 CONTROL SWITCHES l

Nature of the Problem As reported by General Electric Company, Nuclear Equipment Division.(GE-NED),

on May 28, 1981, the CR2940 tandem block control switches used in the Power Generation Control Complex supplied to Grand Gulf Project of Mississippi Power

& Light, have a defect. The switches have contact blocks that came loose during shipment to the site. Vibration during a seismic event could cause a contact block to loosen sufficiently,'further inhibiting proper operation of the switch.

Safety Significance GE type CR2940 switches are used in numerous applications at WNP-2, in NSSS, and in B0P for both safety and non-safety related systems. The above defect has a great potential for compromising the safety of plant operations.

Corrective Action Test and Start-up is presently reviewing the plant electrical drawings to determine which safety related systems use GE CR2940 switches. When defective GE CR2940 switches are found in the safety related systems, a Start-up Deficiency Report (SDR) will be initiated.

This will document the deficient-condition and the necessary corrective action of tightening the bolts which hold the contact blocks together. The review will be completed prior to the preoperational testing of the associated system. Fifty four (54) switches have been identified to date; twenty four (24) of these have been checked and two (2)

~

have found to be defective. The total number of switches in safety.related systems has been conservatively estimated at one hundred (100).
