-Docket No. 99900348/81-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on August 17-21,1981, it appears that certain of your activities were'not con. ducted in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR Part 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfac-torily accomplished."
Nonconformances with these requirements are as follows:
NPS Industries, Inc. Quality Assurance Manual, Section 16.0, Corrective Action, requires that the Manager of Quality Assurance determine the cause of signifi-cant or recurring nonconformances and initiate a Corrective Action Request
-(CAR) to correct and eliminate the recurrence of the nonconformances.
Contrary to the above, the CAR's or other documents related to the adverse condition did not record the f act that the Manager of QA had determined the cause of the nonconfonnity that initiated the CAR.
However, prior to the conclusion of the inspection, the vendor revised the CAR form to require the cause of the deficiency to be recorded, and the Work Procedure No.16.0.1 was revised to describe the method to be used to complete this part of the CAR, therefore, a response to this nonconformance is not required.
