ATTACHMENT 1 RESPONSE TO APPENDIX A Violation 10CFR50, Appendix B,

Criterion IX, states in part, that:

" Measures shall be established to assure that... welding... (is) controlled and accomplished...in accordance with applicable codes, standards, specifications, criteria, and other special requirements."

The Limerick PSAR, Appendix D, paragraph D.4.3,

" Control of Special l

Processes, Tests and Inspections", states, in part, that:

"The vendor shall operate under a controlled manufacturing system and 3

shall have written procedures or instructions for control of...

The pipe drawing DBB-203-1, Revision 3, Note 4.B, states " Grind ID of buttweld flush (where accessible) and OD per detuil

Contrary to the above, on July 14, 1981, vendor shop weld No.

3, on pipe spool DBB-203-1, was not ground flush on the inside diameter.

l Response to Violation NCR 4923 was issued to document the nonconforming ceadition on pipe spool DBB-203-1.

The nonconforming condition was reworked and reinspected in accordance with approved procedures.

The NCR was closed on September 4, 1981.

i A review of all pipe spools subject to PSI /ISI requirements that have been receipt inspected but not installed (130 pipe spools) was performed by the Concractor for identification of similar conditions.

NCR 4971 was issued to document the nonconforming conditions identified during this review.

Actions to resolve this NCR will be completed by November 2, 1981.

Similar conditions that i

exist in pipe spools already installed are being documented as ID reflectors as a result of pre-service ultrasonic inspections now in progress.

Actions Taken to Prevent Recurrence Were:

1.

A training session was given to the Contractor Quality Control Receipt Inspectic" personnel on August 13, 1981 regarding pipe spool specification and drawing requirements.

2.

A letter was sent to the responsible vendor on September 18, 1981, informing him of the nonconforming shop welds and reminded him that specification requirements must be adhered to and requested closer inspection of completed pipe spools.
