w Gilbert / Commonwealth Docket No. 99900525/81-01 NOTICE OF NONCONFORMANCE Based on the results cf an NRC inspection conducted on March 31 - April 2, 1981, it appears that certain of your activities were not conducted in full compliance with your commitments as indicated below.

A.

Criterion V of Appendix B to 10 CFR Part 50, states, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantita-tive or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Paragraph 6.2.3 of the Gilbert / Commonwealth procedure for implementing 10 CFR Part 21 requirements (Procedure For Processing Reportable Events) states in part, "The Cognizant Manager shall conduct a preliminary evalua-tion of the possible Reportable Event.

The appropriate portion of Form j

QAD 600 shall be completed...."

l Contrary to the above, the Cognizant Manager did not conduct a preliminary evaluation, nor complete the appropriate portion of Form QA0 600 for possible Reportable Events SN 002 through SN 016.

The individual who l

did conduct and document the evaluations was not authorized to do so.

Gilbert / Commonwealth took corrective action and implemented preventive I

measures before completion of this inspection, therefore, no written l

response is required.

Section 17.3 of the Gilbert / Commonwealth Topical Report GAI-TR-106, Revision 2A, states in part that, " Designs are prepared, reviewed, and approved by qualified Power Division personnel to assure coverage and/or inclusion of the pertinent requirements.

Completed designs are subjected to a design verification to assure that the design is capable of meeting specified requirements... Errors and deficiencies in the design process are detected during the design verification cycle and/or during the l

performance of Quality Assurance Audits."

Contrary to the above, errors and deficiencies in the design process were not detected during the design verification cycle nor during the perform-ance of Quality Assurance Audits.
