Docket No. 99900746/81-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on March 31 - April 3. 1981, it appears that one of your activities was not conducted in accordarice with NRC requirements as indicated below.
Criterion V of Appendix 8 to 10 CFR Part 50 states:
~" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, or a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instractions, procedures, or drawings shall irclude appropriate quantitative or qualitative acceptance criteria for determinir.3 that important activities have been satisfactorily accomplished."
QA Manual Section 19, paragraph 19.2 states in part, "The Inspector, the Quality Engineer CNC or the N.DE Supervisor, as applicable shall attach or have attached to the nonconforming item a HOLD Label, arrange for segregation where practical, and shall complete Part A of the Non-Conformance 8
Report (NCR)...."
Contrary to the above, 12 nonconforming diffusers, identified by open NCR 14/6.3/125/1 as being located in a certain Hold area, could not be located.
