NOTICE OF VIOLATION Akron General Medical Center License No. 34-03362-02 As a result of the inspection conducted on September 4,1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:

a 1.

License Condition 8.B states that the maximum amount thet yco may possess at any one time under this license is 2 curies of each byproduct material authorized in Group III of Sciedule A, Section 35.1ro of 10 CFR 35.

Contrary to the above, since the date the license was renewed on June 26, 1978, you have received 2.250 curies of molybdenum-99 in the incu of a :fo-99/Tc-99m generator cvery week. These benerators and' molybdenum-99 are authorized in Group III'above.

This is a Severity Level V violation (Supplement VII).

Condition No. 17 of your license requires that licensed material be possessed and used in accordance with the statements, representa-tions,.and procedures contained in certain referenced applications and letters.

The refert nced letter dated June 6,1978, states that the Regulatory Guide 10.8, Appendix F, procedures will be followed ccncerning receipt of radioactive material packages. Appendix F requires that all pack-ages be surveyed at 3 feet and at the surface, in addition to a contamination wipe test.

performed on packages received in the nuclear medicine depar.tment and on 5 shipments of iodine-125 sealed sources received in the oncology department since January 16, 1980.

This is a Severity Level V violation (Supplement VII).

Under the authority of Section 182 of the Atomic Energy Act of 1954 as amended, this response 1
