1 ABNOPMAL OCCURRENCE REPORT Big Rock Point Plant 1.

Report No - A0 8-74, Docket 50-155 2.

Occurrence Date - April 11, lW4 O73 4.

Facility - Big Rock Point Plant 5

Title - Off-Gas Drair Valves Not Properly Adjusted

6. Condition Prior to Occurrence - All conditions since Dace:9er 1W3, include power operr. tion, refueling operation and shutdown.

7 Description - An inspection into the problems associated with the off-gas holdup system integrity was being conducted during the present refueling outage when two valves, CV 4030 and CV 4035, were found not properly seated in the closed position. De valve travel limits were readjusted and pressure testing is new being conducted to determine leakage characteristics of the valves.

Designation of Cause - The adjustments for these air-operated fail-safe valves were not clearly understood by the mechanic during the inspections conducted in 1W3 These valves were imprcperly i

adjusted following reassembly in December lW3 9

Analysis of Occurrence - As reported in the letter fran R. B. Sewell to J. F. O' Leary en January 21,1W4, the on-line off-gas system integrity was still inadequate. These two valve deficiencies con-trib'Ite to a plausible explanation of the failure of that test. De deficiency of valve CV 4035 probably explains the contacdnatica f

experienced in the turbine building during the January 11 test. Ce off-gas holdup pressure had built up to approximately two psig sten i

January 11,1W4 test was terminated due to that high air activity level in the turbine building.

Since the administrative controls as imposed by our letter of June 26, 1W2 have been continuously in effect, we do not feel that an n*e hazard has existed during this protracted test period.

10. Corrective Action - In addition to the measures outliced in I+a 7, Description, we are in the process of developing more detailed procedures for adjustment of this type f air.-operated valve. W on-line holdap test of the off-gas system vill again be cc.xiucted after return to power operation.
