Cenditions Prior to occurrence: Plant crerating at 200 !G. and preparations were being made to conduct the semiannual conta r. ent sir lock leak rate test (T180-01).

Lcreriptien of Occurrence: Inspection of the containment emergency escape 1ccr. at 1200 hours0.0139 days <br />0.333 hours <br />0.00198 weeks <br />4.566e-4 months <br /> on September 17 revealed that the test fixture used to pressurize the locP had teen left installed on the lock with the test fixture valve, used fer venting, in the cpen position. Since June 21,197L, the inner door of the lock has been left open for personnel safety requirements and, thus, the containment has teen cpen to outside atmosphere via a small opening of approxi-

=ately one-nslf inch diancter in the test fixture. The test fixture has been 1: properly installed on the lock since April 29,197k.

7 Desien stien of Arnarcr.t Cause of Occurrer.?e: The pro:edure in use fer

  • he April leak rate tasting (T730-01) called for return of the lock to norn.1 following use of the test equipment, but was not detailed enough to incluse all items to te done to return the lock to nernal. The procedure outlined the installation and use of the test fixture in detail but the fixture ec.iition was overlooked on return to service.

Analysis or occurrence-Except for brief testing periods on the contain=ent isolation valves the contaircent has been open to atmosphere "la the normal ventilation cyttes to the 2h0-foot stack and no conditions callin6 for isolation tsve existed since April 29, 197h. liovever, if such isolation conditiens had occurred with the escape lock inner door open, the contr.ircent integrity require =ents for leak rate vould not have been met. Since the contaircent is under slight negative p essure.during nor:11 operating and shutdown ccnditions, there have been no un=onitored or uncontrollec releasa to the environ =ent.

Further actien to precluda rt:2rres:e vill include revision of the test procedure (T730-1) to ine;ude detailed steps to return to r.ortal operation. Also, all o'at:th dine _ safety-related test procedures and repair procedures will te reviewed and revised to include more detailea steps to return to n:r al operation prior to their next use. Also, General Office re;resentatives will discuss this subject and related catters at future meetings with 316 Rock Point Plant personnel.
