Dear Mr. O' Leary:

(*R~

This letter vill infom yoa of two abnorr '. occurrences which o.. curred during our last shutd.u on November '.-3,1973.

They hy h:ye are described below:

in m a

$g%a Ao-lh-73 m$I:k h The precautions and limitations cection of the plant shut-down targin test procedure requires that neutron monitoring system f;,$

%ft-checks be completed prior to conducting shutdown cargin tests. On Ncvember 3,1973, shutdown margin testing was commenced prior to con-wkj ducting the required neutron c:onitoring system checks. When it vr.c jUn recognized that the neutron ronitoring checks had not been perforced

  • M'y b

tha chutdevn margin testing was halted until the neutron ronitoring

, dMM checks were co=pleted.

The cause of this incident was due to incomplete infomation k

))l exchange at the shift change. The oncening snifc ~.hought that all re-quirecents of the procedure.s and licitations secti< n of the procedure

actual shutdown margin testing. As Str.ted above, it vns Ic.tr dir-aovered that the neutron conitorig:g systen checks had not been cet-pleted.

The Plant 0;$ rating Proc' dures ' Manual' states that,' "The ~

e in-core flux monitors will be recalibrated utilizing flux vires.

Calibrations vill bc.perfomed for approximately every 20 notches

~

of control rod movement or.When the root mean square deviation approaches 155." Howeverb based on a review of the records on.

November 7,1973, the Reactor Engineer discovered that the in-core monitors had not been calibrated as " required.

The Big Rock Point Plant in-core flux monitors are only used to monitor power distributions and detect deviations fran predicted power distributions. The in-core detectors provide no automatic pro-tective functic;.

The Plsnt Review Committee coccluded that the 20-notch limit in the procedures manual is meaningless and will be eliminated as a requirement. The important requirement was concluded to be the root i

mean square deviation of 15 percent. The committee further concluded that a recurrence could best be preverted if the Reactor Engineering l

Department kept a trend log of the root mean square deviations; and.

based upon the trends observed, initiate action to ensure that the in-core monitors are recalibrated p.;or to exceeding the 15 percent devi-atiot. limit.

The Plant Review Committee recoe:mendations are being imple-mented by the plant staff.

Yours very truly, Ralph B. Sevell (Signed)
