1 APPENDIX A NOTICE OF VIOLATION O. S. Department of Interior License No. 50-11901-01 1209-Orca Street Anchorage, Alaska 99501 As a res@t of the inspection conducted on August 5,1981, and in accordance with the Interin Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified:

10 CFR 20.201(b) requires licensees to perfom such surveys as may be necessary for him to assure compliance with 10 CFR 20.105(b).

Contrary to the above requirement, at the time of the inspection, the licensee had not performed surveys of the radioactive materials storage area since September 15, 1972 to demonstrate compliance with 10 CFR 20.105(b).

This is a Severity Level V Violatior- (Supplement VII).

License Condition 17. specifies that licensed materials be utilized in accordance with statements, representations, and procedures contained in applications dated June 24, 1968 and June 5,1973 and application received June 20, 1978.

The June 24,1%8 application, Item 11, requires that CDV-700 survey meters be calibrated every 6 months.

Contrary to the above requirement, at the time of the inspectinn, no records of survey instrument calibrations were available for CDV-700 -

survey meters No. 70740 and No. 50921.

This is a Severity Level VI violation (Supplement VII).

Pursuant to the provisions of 10 CFR 2.201, the U.S. Department of Interior, I,achorage, Alaska, is hereby required to submit to this office within thirty.

l days of the date of this Notice, a written statement or explanation in reply, l

including:

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will ba taken to avoid further items of L

noncompliance; and (3) the date when full compliance will be achies4d.

Consideration may be given to extending your response time for good cause shown. The responses directed by this notice are not subject to the clearance procedures of the Office of Management and Budget as required' by the Paperwork j

Reduction Act of 1980, PL 96-511.
