Appendix A NOTICE OF VIOLATION Waconia Ridgeview Hospital License No. 22-17053-01 As a result of the inspection conducted on August 21, 1981, and in accord--
ance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),
the following violations were identified:
1.
10 CFR 35.14(e) requires that sealed calibration sources be leak tested at six month intervals and that such tests be capable of
' detecting the presense of 0.005 microcuries of radioactive material on the sample.
Contrary to the above, your present method of wipe test analysis is not sufficiently sensitive to detect 0.005 microcuries.
Specifically, it was found during the inspection that your wipe tests are analyzed with your dose calibrator which has a minimum sensitivity of 0.12 microcuries.
This is a repeat item of noncompliance.
This is a Severity Level V violation (Supplement VII).

License Condition Noi 14 requires that licensed material be possessed j
and used in accordance with statements, representations and procedures contained in application dated May 27, 1976, and certain referenced letters.
j Contrary to the requirements of Item No. 12 of the above referenced application, it was learned through statements of licensee representa-tives and review of records, that you failed to assign finger ring badges to individuals handling and administering radiopharmaceuticals to patients.
This is a Severity Level V violation (Supplement VII).

10 CFR 20.L01(b) requires-that you maintain records showing the results of surveys required by 10 CFR 20.201(b), "Eurveys."
Contrary to this requirement, as of the day of the inspection, you 1
failed to maintain records of surveys 'that you performed to assure compliance with.20.301, " Waste Disposal, General Requirement."
This is a Severity Level VI violatiou (Supplement IV).

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or a ffi rma tion.
Consideration may be given to extending your response time for od cause shown.
