This refers to the routine safety inspection conducted by Mr. R. E. Burgin of this office on August 19, 1981, of activities authorized by NRC Byproduct Material Licenses No. 48-13249-02 and No. 48-13249-03 and to the discussion of our findings with Dr. Bahr and members of your staff at the conclusion of the inspection, and with you by telephone on August 24, 1981.
The inspection was an examination of activities conducted under your license as they relate to radiation safety and to compliance with the Commiasion's rules and regulations and with the conditions of your license. The inspec-tion consisted of a selective examination of procedures and representative records, observations, independent measurements, and interviews with personnel.
During this inspection, no items of noncompliance were identified for License No. 48-13249-03 (teletherapy); however, for License No. 48-13249-02 (nuclear medicine) certain of your activities appeared to be in noncom-pliance with NRC requirements, as specified in enclosed Appendix A.
A written response, submitted under oath or affirmation, is required.
