Appendix A NOTICE OF VIOLATION Veterans Administration Ndical Center P.O. Box 1034 3710 S.W. U.S. Veterans Hospital Road Portland, Oregon 97201 License No. 36-01395-01 As a result of the inspection conducted on July 21, 1981, and in accord-ance with the Interim Enforcement Policy, 45 FR 66754.(October-7, 1980),
the following violations were identified:
A.
License condition 13.D states that records of leak test results shall be kept in units of microcuries and maintained for inspection by the Comission.
Contrary to the above requirement, at the time of the inspection, the records of leak tests conducted subsequent to July 20, 1978, were not kept in units of microcuries.
This is a Severity Level VI violation. (Supplement VII)
B.
10 CFR 20.401(a)(3) requires that records shall be maintained in the same units as used in 10 CFR 20 for licensed materials released to the sanitary sewer which are made pursuant to 10 CFR 20.303(d).
Contrary to the above requirement, records of the activity of the licensed material released to the sanitary sewer were recorded in units of radiation levels (mr/hr) instead of activity units during disposals made on Jar.rary 8, 1979 and June 23, 1981.
This is a Severity Level VI Violation.

C.
10 CFR 20.401(b) requires each licensee to maintain records of the result of surveys required by 10 CFR 20.201(b).
Contrary to the above requirement, at the time of the inspection, a record was not maintained of the survey of a patient's room made subsequent to his dis' charge after receiving a therapeutic dose of 100 millicuries of Iodine-131 on December 1, 1978. A telephone conversation with a licensee's representative indicated that the survey had been made but that the record had not been maintained.
This is a Severity Level VI Violation.

(1) the corrective steps which have
.i been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved. Consideration may be given to extend-ing your response time for good cause shown.
The responses directed by this notice are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511. Under the authority of section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
