sq APPENDIX A NOTICE OF VIOLATION Baltimore Gas and Electric Company Docket tio. 50-317 Calvert Cliffs Unit 1 License No. DPR-53 As a result of the inspection conducted on May 4-31, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980) the following violations were identified.
A.
Technical Specification 3.3.3.6, Post-Accident Instrumentation, states in part:
"3.3.3.6 The post-accident monitoring instrumentation channels shown in Table 3.3-10 shall be OPERABLE.
APPLICABILITY: MODES 1, 2 and 5.
i ACTION:
a.
With the number of OPERABLE post-accident monitoring channels less than required by Table 3.3-10, either restore the inoperable channel to OPERABLE status within 30 days, or be in HOT SHUTDOWN within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />."
Table 3.3-10 shows in part the following instruments:
4 "13. PORV/ Safety Valve Acoustic Flow Monitoring....... 1/ Valve."
4 Contrary to the above, the Acoustic Flow Monitor for Safety Valve 201 was inoperable on April 21, 1981, when license amendment 53 requiring its operability was issued, and that Acoustic Flow Monitor was not returned to service within 30 days, and the olant was not shutdown within 30 iIays.
The Limiting Condition for Operaticn was exceeded from May 21, 1981 until May 27, 1981, when Technical 59ecification 3.3.3.6 was amended to allow continued operation until June 1, 1981 with the acoustic monitor involved inoperable.
This is a Severity Level IV Violation:

10 CFR 50.72, Not1fication of Significant Events, states in part:
"(a) Each licensee of a nuclear power reactor licensed under Section 50.21 or 50.22 shall notify the NRC Operations Center as soon as possible and in all cases within one hour by telephone of the occurrence of any of the following significant events and shall identify the event as being reported pursuant to this section:.......(5) Any event requiring initiation of shutdown of the nuclear power plant in accordance with Technical Specification Limiting Conditions for Operation......"
Contrary to the above, no report to the NRC Operations Center was made until two hours and thirty-five minutes after disccvery of extended inoperability of the Safety Vaive 201 Acoustic Monitoring System at 2:00 p.m. on May 27, 1981, an event requiring initiation of shutdown in accordance with Technical Specification Limiting Condition for Operation 3.0.L.
This is a Severity Level VI Violation:

Pursuant to the provisions of 10 CFR 2.201, Baltimore Gas and Electric Company is hereby required to submit to this office within twenty-five days of the date of this notice, a written statement or explanation in reply, including (1) corrective steps which have been taken and the results achieved, (2) corrective steps which will be taken to avoid further violation, and (3) the date when full compliance will be achived.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended thi.i response shall be submitted under oath or affirmation.
