Final Report on 10CFR50.55(e) Item on Inadequate Penetration for Dravo Lug Welds (#41)

Dear Mr. Keppler:

This is Detroit Edison's final report on the Inadequate Penetration for Dravo Lug Welds. The problem was originally reported to Mr. J.E. Konklin of NRC-Region III by Detroit Edison's Mr. H.A. Walker, Supervisor-Construc-tion Quality Assurance on March 24, 1981.

At that tine, it had been noted on a routine inspection of a pipe support that a lug which had been welded to the pipe in the shop by Dravo did not appear to have adequate penetration. This lug as well as other Dravo lugs were to have been full penetration welded to large bore pipe at specific locations. Subsequent inspection of other Dravo installed lugs indicated that the problem existed at a number of other locations. The problem had been noted at nine (9) lug locations throughout saveral systems at the Fermi 2 Plant. A renoved lug indicates that there is a lack of penetration in the entire length of the lug on the centerline. It was felt that a failure of these lugs would damage safety related systems at the Fermi 2 Plant.

Ultrasonic testing of the Dravo lugs has been completed. DDR's have been written on the defective lugs. All defective lugs on ASME Class I piping have been replaced. Defectivc lugs for ASME Class II and III piping have been evaluated to determine acceptabj'.ity. Class II and III lugs requir-ing replacement or repair have been identified. Required repairs are in process and should be completed in the near future.
