As a result. of the inspection conducted on August 4, 1981, and in accord-ance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),
the following violations were identified:
License No. 12-00663-03 1.
License Condition No. 14 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated June 1, 1977; and letters dated October 13, 1977, November 11, 1977, and February 1,1978.
The application dated June 1,1977. names a specific individual as radiation protection officer.
Contrary to the above, the individual designated as your rsdiation protection officer was changed and you failed to amend your license to reflect this change.

License Coniition No. 14 requires that licensed material be possessed and used in accordance with statements, representations, and procedures contained in application dated June 1, 1977; and letters dated October 13, 1977, November 11, 1977, and February 1, 1978.
The license application dated June 1, 1977, states that a low level G-M survey meter shall be used to perform daily surveys.
l Contrary to this requirement a low level G-M survey meter was not avail-able to perform daily surveys from July 29, 1981 to August 4, 1981.
This is a Severity Level V violation (Supplement VII).

10 CFR 20.203(e) requires that a room in which licensed material is used or stored shall be posted with a sign bearing the radiation caution symbol and the words, " Caution Radioactive Material."
Contrary to this requirement, it was observed at the tica of the inspection that the licensee failed to post a " Caution Radioactive Material" sign in an area which is used for the storage of byproduct material.
This is a Severity Level VI violation (Supplement IV).

Contrary to the above requirement, it was determined through state-j ments by licensee representatives and the NRC inspectors review of records that this condition is rot being met.
Specifically, the above records were not available for review on the day of this inspection.
This is a Severity Level VI violation (Supplement VII).

this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) cor-rer.tive action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of-the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath j
