ENCLOSURE FAILURE BY ITT GRINNELL TO USE PROPER TESTING METHODS 10 CFR50.55( e)
HTRD-50-51d/81-08, -519, -520, -521/81-07 PBRD-50-553/81-10 REPORT NO. 3 (SUPPLEMENTAL INTERIM)
Description of Deficiency This deficiency involved fittings such as piping tees and elbows on 23 spoolpieces.
The pipe sizes invcived are 12, 20, and ?4 inches. During a review of material test reports for fittings on the spoolpieces, it was noted by ITT Grinnell that certal" items which should have been examined by penetrant or magnetic testing were actually examined by ultrasonic testing (two circumferential directions). This testing is not in accordance witi) ASME P&PV Code, 1074 Edition, Winter 1975 Addenda, Subarticle NB2551(b), which requires penetrant or magnetic particle testing of fittings for Nucicar Class 1.
Interim Progress TVA has reviewed the subject nonconformance report along with ITT Grinnell's proposed disposition, and as a result, has determined the following corrective action, are required to accept the fittings:
(1) Fittings welded to spools must pass magnetic particle examination of all external surfaces and all accessible internal surfaces in accordance with NB-2551(b) (ASME, Winter 1975 Addenda).
(2) Because the fitt, ings were fabricated frcm a number of different heats, a sampling of inaccessible inside surfaces of a selected number of fittings shall be examined to assure the quality of all the heats involved. The selected fittings shall be removr.d from the spool pie.as by sectioning.
A minimum number of two from each heat shall be examined.
