Shelton Senior Vice President- Nuclear c/o Toledo Edison Company 300 Madison Avenue Toledo, OH 43652 Dear Mr. Shelton: SUBJECT: ROUTINE RESIDENT INSPECTION AT DAVIS-BESSE (50-346/94004(DRP)) This refers to the inspection conducted by Messrs. S. Stasek, and J. M. Shine of this office on March 3, 1994, through April 15, 1994. The inspection included a review of activities authorized for your Davis-Besse Nuclear Power , Station. At the conclusion of the inspection, the findings were discussed - with those members of your staff identified in the enclosed report. Areas examined during the inspection are identified in the report. Within these areas, the inspection consisted of' selective examinations of procedures and representative records, interviews with personnel, and observation of activities in progress.. No violations of NRC requirements were identified during the course of this inspection. The operating crews performed very well during this period with the unit continuing to be operated in a controlled, conservative manner. Overall, your engineering groups continue to provide adequate support to the rest of the onsite organization. Of particular note, is the establishment of an action plan to comprehensively evaluate testing and setup of your air operated valves. However, engineering evaluations performed in support of temporary lead blanket shielding were found to be weak in some cases, weaknesses were evident in problem resolution and documentation associated with the lube oil reservoir level switch failure, and we are continuing to review apparent problems associated with the hydrogen recombiner provisions.to control combustible gases following an accident. In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter and the enclosed inspection report will be placed in the NRC Public Document Room.
