ATTN:

Plant Manager
Big Rock Point Nuclear Plant

1
Dear Mr. Donnelly:
This refers to the routine safety inspection conducted by Messrs. R. Leemon,
C. Brown, and S. Orth of this office from March 17 through May 5,1994.
The
inspection included a review of activities at the Big Rock Point Nuclear
facility authorized by NRC Operating License No. DPR-6.
At the conclusion of
the inspection, the findings were discussed with those members of your staff
identified in the enclosed report.
Areas examined during the inspection are identified in the report.
Within
these areas, the inspection consisted of a selective examination of procedures
and representative records, observations, and interviews with personnel.
The
purpose of the inspection was to determine whether activities authorized by
the licensee were conducted safely and in accordance with NRC requirements.
The results of this inspection showed that management efforts to improve
safety focus were effective.
This was best exemplified in your efforts to
correct minor system leaks earlier than originally planned.
Strengths were
noted in operators' responses during the startups, shutdowns, and response to
a plant transient.
In addition, maintenance and engineering efforts to
support operations and the two outages showed significant improvement.
Weaknesses were evidenced by the need to' conduct two outages to effect plant
repairs and the three examples of failure to follow procedures.
We do note
that as a result of the work performed during these outages plant performance
has been significantly improved, with primary leakage and overall water usage
the lowest it has been in ten years.
Certain of your activities appeared to be in violation of NRC requirements, as
specified in the enclosed Notice of Violation (Notice).
The violation was for.
failure to follow procedures. Our review of the violation determined that it
was not of major safety significance.
Two additional violations were also
identified; however, since the requirements of 10 CFR Part 2, Appendix C,
Section Vll.B were met, they were not cited.
