SUBJECT:

REACTIVE REVIEW 0F POTENTIAL TAMPERING EVENT

This refers to the inspection conducted by Mr. J.

.R. Creed of this office

between February 4 and April 21, 1994.

The inspection included a review of

authorized activities for your LaSalle Nuclear Power Plant.

Areas examined

during the inspection are identified in the report. ' Within these areas, the

inspection consisted of selective examinations of records, interviews with

personnel, and observations of plant equipment.

No violations of NRC requirements were identified during the course of this

inspection.

We concluded, however, that foreign material was intentionally introduced into

one of your lubricating systems. Although unable to be proven conclusively,

it appears more likely than not that this was the result of poor work

oractices rather than a malevolent act. However, despite early indications of

intentional tampering, your Problem Identification Program and event screening

process were apparently not initially effective in highlighting this issue.

Consequently, adequate security evaluation of this matter was delayed.

The

corrective action taken to address the first incident by your site staff was,

l

l

by your own . evaluation, inadequate.

Clear initial indications of intentional misuse or tampering with systems ~ need

be addressed more aggressively and more timely, with a team capable of

evaluating both the operational and security aspects of the facts. Better

-coordination between site operating staff, security specialists and corporate

investigators should be developed.

This might include criteria for action, a

method or procedures providing guidance for determining whether an abnormal

plant condition could have a reasonable explanation or could be intentional.

A stronger questioning attitude may help in developing accurate evaluation of

potential tampering events sooner.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of
