SUBJECT:

NRC INSPECTION REPORT NOS. 50-348/94-07 AND 50-364/94-07

This refers to the inspection conducted by T.M. Ross of this' office on March

14 - April 10, 1994.

The inspection included a review of activities

authorized for the Farley Nuclear Plant.

At the conclusion of the inspection,

and at various times during the inspection, findings were discussed with those

members of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the report.

Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with per.sonnel, and observation of

activities in progress.

Based on results of this inspection, certain aspects of your activities

appeared to be in violation of NRC requirements, as specified in the enclosed

Notice of Violation (Notice).

These violations involved ineffective

corrective actions to address past problems regarding the approval of

scaffolding used in the vicinity of safety-related equipment, and the failure

to follow the applicable Technical Specifications Action Statement during

surveillance testing of Nuclear Instrumentation System Power Range channels.

The fact that this violation of Technical Specifications existed for an

extended period of time is of particular concern to the NRC.

You are required to respond to this letter and follow the instructions

specified in the enclosed Notice when preparing your response.

In your

response, you should document the specific actions taken and any additional

actions planned to prevent recurrence of these violations.

Furthermore, the

generic and programmatic aspects of surveillance activities should be

addressed.

After reviewing your response to the Notice, including proposed corrective

actions and the results of any future inspections, the NRC will determine

whether further NRC enforcement action is necessary to ensure compliance with

NRC regulatory requirements.
