February 4, 1994 HEMORANDUM FOR:

Bruce A..Boger, Director Division of Reactor Controls.

and Human Factors Office of Nuclear Reactor Regulation FROM:

Gary M. Holahan, Director l

Division of Safety Programs Office for Analysis and Evaluation of Operational Data

SUBJECT:

AE00 SPECIAL STUDY, "ROSEMOUNT TRANSMITTERS - LOSS OF FILL-OIL FAILURES" I

This memorandum provides our response to your request for AE00 assistance in addressing a recommendation made by the Rosemount Transmitter Review Group (RTRG).

Your November 22, 1993 memorandum stated that "the review of NPROS failure data every 6 months for the next two years will, identify whether or not a significant increase in the number of reported fill-oil failures has occurred.

In this manner plants which may experience a significant number of failures will be determined."

Following receipt of your request, P. O'Reilly and J. Houghton of my staff met with J. Mauck and D. Spaulding of your staff to discuss and clarify the details of our support effort. This agreed upon support is in the form of a special study, engineering data, and failure data.

The AEOD special study documented in the enclosure provides the number of failures, failure rate per year and trending for the Rosemount transmitters for a five year period (07/01/88 - 06/30/93), with the latter half (04/01/91 -

06/30/93) as the initial update.

Further updates are planned on a semi-annual basis, as requested.

The hardcopy NPROS counts for number of components and hardcopy NPRDS component master failure reports, with engineering data and failure data for the specific Rosemount transmitter fill-oil failures during the 5 year period, are oroorietarv information and have been transmitted to your J. Mauck by separate transmittal.

In addition, a gnportetary information diskette was also included in the separate transmittal, which contains files for the component counts, failure masters, and a ' file identifying coded plants in the study.
