COMBINED INSPECTION REPORT NOS. 50-277/91-03; 50-278/91 03

This letter transmits the findings of the routice resident safety inspection conducted by Messrs.

J. J. Lyash, R. J. Urban, F. P. Bonnett, and L. E. Myers from January 8 through February 11,

1991, at the Peach Bottom Atomic Power Station, Delta, Pennsylvania. The inspectors based

these findings on observation of activities, interviews and document reviews, and discussed them

with Mr. J. F. Franz of your staff.

Based ca the results of this inspection, one of your activities appeared to be in violation of NRC

requirements, as specified in the enclosed Notice of Violation. This violation involved the

operation of controls that directly affect reactivity by a non licensed operator. The non-licensed

operator was not in the presence of a licensed operator or a participant in a license training

program. Although Section 2.201 requires you to submit to this office a written statement of

explanation within 20 days of your receipt of this Notice, we note that your immediate and

planned corrective actions, as documented in this report, adequately address this issue. There-

fore, no response with respect to this matter is required. If our understanding of your corrective

actions as stated in this report is not correct, please notify us immediately.

During this inspection, we observed refueling activities and found them to be generally well

coordinated and conducted. However, subsequent to this inspection, several fuel handling errors

occurred and were the s6 ject of a management meeting in the Region 1 Office on February 25,

1991. These problems and your corrective actions will be reviewed and documented in Inspec-

tion Report Nos. 50-277/91-08; 50-278/91-08
