Subject: NRC Region I Inspection No. 50-336/91-01

This letter transmits the NRC report of a routine resident inspection conducted by Mr. Peter

Habighorst of Region I on January 7 - February 16, 1991, at Millstone Unit 2.

At the

conclusion of the inspection, the findings were liscussed with Mr. J. S. Keenan and other

members of your staff.

Areas examined during the inspection are described in the enclosed report. Within these areas,

the inspection ' consisted of performance observations of ongoing activities, independent

.

,

verification of safety equipment status and design configuration, interviews with personnel, and

review of records. Overall operation of the facility was concluded to be satisfactory.

Although not. cited as a violation during this inspection, we are concerned with the poor

communications and breakdown in maintenance work controls which immediately preceded the

reactor _ trip on January 10th. While erratic operation of a main turbine EHC pump was a

contributing factor in the trip, experienced mechanics prematurely began unauthorized,

unsupervised, and unisolated replacement of a filter without an adequate pre job bricting. -We

will focus future inspection followup on these aspects which are critical to effective and safe

maintenance.
