Dear Mr. Young:

As a follow-up to our recent telephone convenations, I have enclosed copies of our draft uniform manifest forms with instructions.

The development of these forms represents an extension of a proposed Nuclear Regulatory Commission (NRC) rulemaking on " Low-Level Waste Shipment Manifest Information and Reporting," and has been undertaken following requests from the Host State Technical Coordinating Committee and the Low-Level Waste Forum.

With the intention of assuring that the formats of these manifests are practical and that the instructions are clear, we have been soliciting individual comments from a number of persons including waste generators, disposal facility operators, and Agreement State regulators. Thus, your views and those of Mr. Carpenter would be appreciated.
