Reed Senior Vice President Opus West III 1400 Opus Place Downers Grove, IL 60515 -Gentlemen: This refers to the routine resident inspector safety inspection conducted by Messrs. T. E. Taylor, J. Shine, R. Bocanegra, T. Ploski, and D. Jones of this office from December 16, 1990 through February 2, 1991, of activities at the Quad Cities Nuclear Power Station, Units l' and 2, authorized by NRC Operating -License Nos. DPR-29 and DPR-30, and to the discussion of our' findings with Mr. R. L. Bax and others of your staff at the conclusion of the inspection. The enclosed copy of our inspection report identifies areas examined during . the_ inspection. Within these areas, the inspection consisted of a selective examination of procedures and representative records, _ observations, and interviews with personnel. _ No violations of NRC requirements were : identified during the course of this ' inspection; however, I am concerned with the personnel errors that occurred -in .the preparation of the out-of-service discussed in paragraph 5.b of the inspection report. These errors are similar in nature to the errors that occurred-with respect to the communications center during the January 24, 1991 ever.t at Quad Cities Station (See Inspection Report 50-254/91006). They are significant in that they represent failures to control plant systems. You- are requested to investigate the causal factors regarding these errors and - respond in writing within 30 days of the date of this . letter - Because of the similarity, you may include your response with the response to the issues described in Inspection Report'50-254/91006. In accordance with 10 CFR 2.790 of-the Commission's regulations, a copy of -this letter, the enclosures, and your response to this letter will be placed in the:NRC Public Document Room. .
