Fire Suppression Systems - Nos. 1 and 2 Diesel Fire Pumps -
This report was initiated by Incident Report 83-007.
CONDITIONS PRIOR TO OCCURRENCE:
Unit 1 - Mode 5 - RX Power 0 % - Unit Load 0 MWe.
Unit 2 - Mode 1 - RX Power 45 % - Unit Load 400 MWe.
DESCRIPTION OF OCCURRENCE:
At 0514 hours0.00595 days <br />0.143 hours <br />8.498677e-4 weeks <br />1.95577e-4 months <br />, January 6,1983, No.12 Station Power Transformer was de-energized for planned maintenance associated with a current refueling outage. Due to the resulting loss of power to the Nos. F and G Group Buses, the automatic starting feature of both Nos. 1 and 2 Diesel Fire Pumps was disabled. The manual starting capability remained operable, and an operator had been stationed locally to: start the pumps if required. The fire pumps were declared inoperable, and Technical Specification Action Statement 3.7.10.lb was entered.
Due to oversight, prompt notification of the NRC was not performed until 0217 hours0.00251 days <br />0.0603 hours <br />3.587963e-4 weeks <br />8.25685e-5 months <br />, January 8, 1983. Subsequent reporting was also overlooked until January 25, 1983, when written confirmation was transmitted.
APPARENT CAUSE OF OCCURRENCE:
As noted, the automatic starting feature was disabled for planned maintenance. The event apparently involved a number of independent failures to identify the occurrence as requiring prompt notification and 14 day followup report. For this reason, it is likely that human factors contributed to the oversights.
Investigation revealed that fire pump reporting requirements are contained in the action statement text and not in the summary of reporting requirements in Technical Specification Section 6.0.
Personnel involved in the reporting process are therefore not exposed to the requirements on a routine basis. Finally, inoperabil'ity of other safety related equipment due to planned maintenance and within the limits of the action statements is not normally reportable.

ANALYSIS OF OCCURRENCE:
The operability of the fire suppression systems ensures that adequate fire suppression capability is available to confine and extinguish fires occurring in any portion of the facility where safety related equipment is located. The collective capability of the fire suppression systems is adequate to minimize potential damage to safety related equipment and is a major element in the facility fire protection program.

With both fire suppression pumps inoperable:
- 1. Establish a backup fire suppression water system within 24
, hours, and
- 2. Submit a Special Report in accordance with Specification 6.9.2; a) By telephone within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />, b) Confirmed by telegraph, mailgram or facsimile transmission no later than the first working day following the event, and
- c) In writing within 14 days following the event outlining i
the action taken, the cause of the inoperability and the pinns and schedule for restoring the system to 4 operable status.
As noted, the fire suppression capability was maintained, and the event involved no risk to the health and safety of the public. Due to the increase in administrative controls associated with the 4
planned maintenance, the occurrence did not constitute operation 4
in a degraded mode per Technical Specification 6.9.1.9b.
CORRECTIVE ___ACTJRE :
Maintenance on No. 12 Station Power Transformer was completed, and power was restored to the fire pump automatic starting feature. The pumps were satisfactorily tested and were declared operable; Action Statement 3.7.10.lb was terminated at 0001 hours1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />, January 7, 1983.

directly involved were counseled concerning the requirements of the i
action statement. The problem will be generally identified to shift operations personnel by weekly directive. Finally, a review of the Technical Specifications will be perf ormed to determine improvements in special reporting requirements from the human factors standpoint.
