APPENDIX A Westinghouse Electric Corporation Docket No. 99900006/82-02 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on November 16-18, 1982, it appears that certain of your activities were not conducted in accordance with NRC requirements as indi:ated below:

Section 6 of the Westinghouse, Nuclear Fuel Division (W-NFD), Quality Assurance Program Plan, WCAP-7800, Revision SA, states in part:

"Throughout the design, manufacturing, and inspection phases, the activities and operations affecting the quality of the fuel assemblies and core components are controlled through the use of approved drawings, specifications, instructions, and procedures."

Honconformances with these requirements are as follows:

A.

Paragraph 3.3.1 and Table 1 in W-NFD Specification No. NFP 31008, Revision 17,

" Seamless Zirconium Alloy Tubing," require procedures to be submitted for approval by the purchaser on the outside surface finish process, inside surface finish process, chemical composition testing, and tensile testing.

Contrary to the above, there was no evidence of the submittal to and approval by the purchaser of the procedures for the outside surface finish process, inside surface finish process, cilemical composition testing, and tensile testing.

B.

Paragraph 2.1 in W-NFD Specialty Metals Plant Quality Procedure QC-300, Revision D, " Final Inspection System for Zircaloy Tubing," states in part, "Each Zircaloy tube is manufactured to meet the requirements of the latest revision of the applicable customer specificatica... as listed on QCF-3003."

Contrary to the above, manufacturing at the time of this inspection did not have the latest revision of Customer Specification No. NFP 31008 listed on the QCF-3003 form in reference to Order No. 548H20313.

C.

Paragraph 3.2 in W-NFD Specification No. NFP 31008, Revision 17, " Seamless Zirconium Alloy Tubing," states, "The identity of all material with respect to ingot melt number and lot number (as defined in Section 4.3.1) shall be maintained at all stages of manufacture."

Contrary to the above, the identity of some material with respect to ingot melt number and lot number was not maintained at all stages of manufacture as evidenced by:

Two tubes of lot No. F73 2266 were found to have been mixed with lot No. F73 2267 which is from a different ingot melt number and heat treat lot number. These two tubes had been reworked on traveler card G 13448, and 2.

In addition to the above, another tube in lot No. F73 2257 could not be accounted for by comparing inspection records with the actual piece count of the lot.

D.

Paragraph 5.2 in W-NFD Specialty Metals Plant Quality Assurance Program Manual, Revision XI, states in part, "Where applicable, procedures include the following.... Action required when nonconforming material or out-of-specification condition is detected.

Disposition of nonconforming material...."

Contrary to the above, the quality procedures did not include the practice and application of "T tags" which are used to make engineering dispositions of nonconforming material.

E.

Paragraph 3.2.1 in W-NFD Specialty Metals Plant Quality Procedure QC-103, Revision H, " Corrective Action System," states, "If, during inspection, the inspector records rejects and/or scrap above normally expected levels, with the following information for above normal reject / scrap (plying himform QCF or above the designated levels, he notifies his supervisor sup Contrary to the above, form QCF-1030 information has not been supplied by inspectors to their r jpervisors, although above normal reject rates have occurred.
