'Ibe cause of this violation was a failure of a&instrative controls.
Ihe surveillance test prmedure (STP) that chec!m the operability of the source range neutron flux monitors failed to address audible indication. This STP has since been corrected. Additionally, a check-list has been developed for entry into Mode 6 to ensure that all teclo-nical specification requirements am met, 1
