APPEfIDIX A tt0TICE OF VIOLATIO'l Washington Tablic Power Supply Systen Docket flo. 50-508 P. O. Box 1223 Construction t'ermit flo. CPPR-154 Elma, Washington 98541 As a result of the inspection conducted on Decenber 13-17, 1982, and in accordance with the NRC Enforcement Policy, 47 FR 9037 (March 9,1982), the following violation was identified:

10 CFR 50 Appendix B, Criterion V states, in part, that:

" Activities affecting quality shall be prescribed by... procedures...and shall be accomplished in accordance with these... procedures." Criterion XVII of Appendix B states, in part, that: " Sufficient records shall be naintained to furnish evidence of activities affecting quality."

Paragraph 17.1.5 of the quality Assurance Progran documented in approved PSAR Deviation No. 26-UP, states, in part, that " Contractors and vendors, including Ebasco...are required to have written... procedures...which govern their quality related activities...."

Paragraph 17.1.17 of the Quality Assurance Program documented in approved PSAR Deviation No. 26-UP, states, in part, that, " Quality Assurance records are utilized to verify that activities affecting quality have occurred, and that they have been perforned in compliance with specified requirenents."

The AS'1E Boiler and Pressure Vessel Code,Section III,1977 Edition including addenda through Sumer-1978 Figure NC-4427-1 requires the presence of a 1/16-inch gap in socket-welded connections before welding.

Contrary to the above, Peter Kiewit Sons Company is presently installing h

safety-related socket-welded pipe fittings without a procedure which describes the methods to assure that the required 1/16-inch gap exists j

before welding. Records are not established to provide Ujective evidence that the required 1/16-inch gap is present in socket welded Q

connections.

j This is a Severity Level V Violation (Supplement II).

Pursuant to the orovisions of 10 CFR 2.201, Washington Public Power Supply System is hereby required to submit to this office within thirty days of the date of this notice, a written statenent or explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) coirective steps which will be taken to avoid further itens of noncompliance; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause shown.
