Dear Mr. Haynea:
This letter is to confirm our verbal notification to your Mr Archittel of the discovery that
. Ralph the auxiliary feedvater regulating valves failed to the open position thereby rendering the PSAR steam line break analysis invalid.
At 1030. on 1-12-83 while im Mode 3. an air line supplying instr==aat air to the auxiliary feedvater regulating valves I/Ps became diacon-nected.
Locs of air to these 1/Ps caused the auxiliary feedwater regulattaa valves (AFW-4511 and AFW-4512) to fail open.
in the event of a steam line break, auw111ary feedvater actuation on Therefore, low steam generator level would result in prompt auxiliary feedwater flow to the ruptured steam generator.
This would be contrary to the stema line break analysis presented in Section 14.14 of the PSAR which assumes a three (3) minute time delay before auxiliary feedwater reaches the ruptured steam generator.
Consequently. the event is reportable per Technical Specification 6.9.1.8.e.
line was repaired at 1125 terminating the event.
