with starter coils for the containment cooling fans. This problem was considered reportable under the requirements of-10CFR50.55(e).

To the best of our knowledge, this problem has not been identified to the Nuclear Regulatory Commission pursuant to 10CFR21.

DESCRIPTION .

-On July 20, 1981 Ebasco Start Up identified a problem with the starter coil for the containment cooling fans. Once energized, the starter; coils were burning up with regular frequency which could be attributed to an installation

-problem since none of the coils analyzed (as described below) were-found to be defective. The problem affected twelve (12) size 5 coils:in1 anf motor control centers 3A317-S 2M, 3A317-S 3M, 3B317-S 2M.and.3B317-S 3M.

SAFETY-IMPLICATIONS The containment cooling fan starter coil! failures have been characterized as a common mode failure. Had the coils failed, the cooling fans would be un-available for post-accident containment heat remcval, thus violating the requirements of GDC 38 of 10CFR50, Appendix A. The incident could have adversely affected the safety of the plant if left uncorrected.

CORRECTIVE ACTION Non-Conformance Report W3-2845 was initiated to track and document corrective action. The original coils were removed, and twelve replacement coils were installed. Work was completed and accepted on June 8,1982; NCR W3-2845 was closed on June 25, 1982.

In order to determine if coil failure was a generic problem, six of the replacement coils were removed and returned to ITE-Gould for analysis.

Gould Brown Boveri Co. tested the coils, and found no defects. Their conclusions show that there is no generic problem with the coils.

Corrective action to replace affected coils was completed on November 29, 1982 and no subsequent failures have been experienced. This report is submitted
