It describes defects in 'the

- Spring Return Actuators.:resulting in replacement of the-spring' guide sleeves.

-This problem is considered reportable:under the requirements of 10CFR50.55(e). :

This problem was identified to the USNRC by G.H. Bettis ' Corporation under 10CFR23..

DESCRIPTION In November 1980, during testing by thel.Hd Bettis Corporation _in'their R &-D G

Laboratory, a defect was discovered in the N521C-SR/N522C-SR (Spring Return)-

Actuators supplied by Jamesbury Corporation,LPO No. NY-403539, and Fisher Controls Company, PO No. NY-403483. Bettis'found that the sp' ring guide sleeve shifting internally could prevent the movement of.the actuator. The failure to actuate ~

l could only occur in the normal power ' stroke, and if the "0" degree position'of' the: valve is actually past the normal "0" of the actuator. These actuators are'

.used.on valves-in the component cooling water and HVAC systems.

i SAFETY IMPLICATIONS l

r Further evaluation since.the Interim Report has indicated that this deficiency I

on the component cooling water isolation valves to the Reactor Coolant Pumps (3CC-B188A1, 3CC-B189B1, 3CC-B190A2, and 3CC-B191B2) would not create a safety i

haza.d.

The deficiency is, however, still considered reportable due to its j

effect on HVAC control room isolation valves (3HV-B169A, 3HV-B170B, 3HV-B177A,

~

3HV-B178B, 3HV-B171A, 3HV-B172B). This deficiency could have caused these

~

valves.to fai11to isolate.

CORRECTIVE ACTION IMPLEMENTED

+

The corrective action for this significant construction deficiency involved the reworking of the above mentioned valve actuator assemblies.

i Nonconformance Report W3-3058, along with service forms #81-11-235, 81-11-236, 82-4-49, 82-12-122, and G.H. Bettis-Corporation Corrective Action Report No. 0011 were generated to instruct, track, and document corrective action.

l Corrective action was completed under the direction of the Bettis-representative, using'the above mentioned NCR, service forms, and corrective action report.

' The corrective action, service forms, and nonconformance report documentation have j

been reviewed.. accepted, and closed as of December 28, 1982.
