1.
Background:
The Crosby lift indicating switch assemblies (N orifice, serial No. 0001 and R orifice, serial No. 0021) and the Crosby set pressure verification indicating device (no model/ serial number) were to be environmentally tested to meet the requirements of the LOCA/HELB accident conditions at their end-of-qualified-service life and to demonstrate the adequate capability of performing their safety-related functions.
Crosby Valve and Gage Company met with the NRC staff (Messrs. P. De Benidito, J. Lombardi, and J. Zudnas, NRR Mechanical Equipment Qualification Group) on August 21, 1980, Bethesda, Maryland, and submitted the initial Crosby Qualification Specification T-16340, Revision 0.
Wyle Laboratories since developed two approved qualification test plans (45391-1, Revision B, dated December 1980 and 45609-1, dated May 20,1981) from the Crosby qualification specification. The purpose of this inspection was to assure that the LOCA/HELB qualification tests were conducted in accordance with these test plans / procedures and to review documented test records completed by Wyle personnel for acceptance within the prescribed margins outlined.
2.
Results of Prestressing Qualification Tests and LOCA/HELB Qualification Test Setup:
a.
The NRC inspector reviewed the ancmalies which occurred during the prestressing qualification tests conducted in accordance with the test plans. During the seismic prestressing qualification, performed at Wyle Laboratories in December 1981, one lead switch (B contact) failed in one redundant train of the lift indicating switch assembly (R orifice, serial No. 0021). A report from Crosby, which will address this technical issue, will be reviewed during a future inspection.

The NRC inspector reviewed the anomalies of the operational test setup for the LOCA/HELB environmental qualification test.
All reed switches to the lift indicating switch assemblies (N orifice, serial No. 0001 and R orifice, serial No. 0021) had indicated change'of state.
The additional 100 feet of cable lead length used, instead of the original 20 feet prescribed in the specifi-cation, was suspected.

Nos. 0001 and 0021) were returned to Crosby Valve and Gage Company for analysis by X-ray techniques. The analysis detected no mal-function of the devices. A report from Crosby, which will address this technical issue, will be reviewed during a future inspection.
3.
LOCA/HELB Environmental Qualification Test:
a.
On August 10, 1982, this test started after obtaining the first qualification test profile (additional peak transient-margin ramp) required in NUREG-0588, Revision 1, and IEEE-STD-382-1980. Due to failure of test apparatus (e.g., electrical Kapton lead wire degradation, as evidenced by the flaking off of lectrical insula-tion), the test was ordered shut down by the Crosby respresenta-tive after completing the first 24-hour period of the test.
Due to the lead wire degradation during this period, switch C of serial No. 0021 would not return to ;ts normal open position, and switches C and F of serial No. 0001 would not respond.
l b.
On August 11, 1982, the chamber head was removed and this anomaly was evaluated and properly dispositioned. Teflon insulated AWG #16 lead wires with Raychem nuclear splices (type WCSF-N with heat-shrinkable polyolefin) were installed, replacing all of the original Kapton insulated type lead wire used inside the test chamber.
c.
On August 17, 1982, the second transient ramp and the LOCA/HELB time-temperature / pressure test profile started at 10:30 a.m.
Since the first qualification test profile (additional transient-margin ramp) had been run on August 10, 1982, Crosby did not intend to rerun that profile.
The second ramp was started with switch C of serial No. 0001 totally inoperable, and switch E of serial No. 0001 actuating with an offset of.065 inch.

PAGE 4 of 4 No. 0001 were not functioning. Switches B and F of serial No. 0001 were double actuating and switch E was still actuating with an.065 inch offset. Switches C and F of serial No. 0021 had shorted at an operating voltage of 30 volts, but switch C

resumed normal operating characteristics at 6 volts. ' Switch F remained shorted with the reduced operational voltage.
A report from Crosby, which will address the technical issues of paragraphs 3a-c, will be reviewed during a future inspection.
4.
Inspection Results:
Within the scope of this inspection, the NRC inspector determined that; a.
Qualification test plan / procedures met the requirements of NUREG-0588, Revision 1.
b.
Test and test activities performed were in accordance with detailed test procedures.
c.
All necessary information and data were collected to allow evaluation of test results and were properly recorded and documented.
d.
The installation of test specimens in the environmental test chamber was in accordance with the qualification test plan /
procedures.
e.
Notice of Anomalies were prepared in a timely manner, properly documented, and properly dispositioned, f.
The lift indicating switch assemblies (N orifice, serial No. 0001 and R orifice, serial No. 0021) did not meet the acceptance criter.ia as outlined in the qualification test plan, 45391-1, Revision B, during each of the LOCA/HELB tests of August 10-12, 1982, and August 17-20, 1982.
g.
The Crosby set pressure verification indicating device (no model/ serial number) met the acceptance criteria out-lined in the qualification test plan 45609-1 Revision 0, and met the requirements of IEEE 323-1974, 344-1975, 382-1980, and NUREG-0588, Revision 1, Category I.
