APPENDIX A NOTICE OF VIOLATION Mississippi Power & Light Docket No. 50-416 Grand Gulf License No. NPF-13 As a result of the inspection conducted on July 19 - August 13, 1982, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9,1982), the following violations were identified.
A.
Technical Specification 6.2.2.f limits operator work time to 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> in any 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> period, or 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> in any seven day period without previous authorization by the plant Superintendent and documentation of the basis for exceeding these guidelines.

On July 16-17, 1982 the same operator' exceeded 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> work time in a 48 hour5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br /> period without prior approval and documentation.
This is a Severity Level IV Violation (Supplement I.).
B.
Technical Specification 6.8.1 requires written procedures be established, implemented and maintained.

Contrary to the above, the requirements of administrative procedure 01-S-06-4, Revision 3, Access and Conduct in the Control Room, paragraph 6.4.1 for the operations superintendent to provide a control room access list to the security supervisor and to maintain the access list in the shift superintendent offices was not met in that on August 5,1982 the security supervisor had not received an access list nor was an access list was being maintained in the shift superintendent office.

Contrary to the above, the requirements of operations procedure 02-S-01-5, Revision 3, Shift Logs and Records that a sunnary of major evaluations performed an a given shift be logged was not met in that for the midnight shift on August 5, 1982, there were no entries to indicate fuel loading was in progress or that surveillance testing was performed, and for the midnight shift on August 6, 1982, there were no entries to indicate surveillance testing was performed.
This is a Severity Level V Violation (Supplement I.).

which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. ConsiderJtion may be given to extending your response time for good cause shown.
