APPENDIX A NOTICE OF VIOLATION Tennessee Valley Authority Docket Nos. 50-259, 260, & 296 Browns Ferry 1, 2, & 3 License Nos. DPR-33, 52 & 68 As a result of the inspection conducted on July 30 - August 10, 1982, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9,1982), the following violation was identified.
10 CFR 50, Appendix B, criterion V, as implemented.by TVA's QA Topical Report, TVA-TR75-1, requires activities affecting quality be prescribed by procedures appropriate to the circumstance and accomplished in accordance with those procedures.
Contrary to the above, the requirement for procedures appropriate to the circumstances was not met prior to July 30, 1982, in that Surveillance Instruction 4.7.C, Secondary Containment, did not account for inleakage to the secondary containment zone under test that results from operation of the normal reactor building ventilation system in adjacent secondary containment zones. This condition allowed masking of the true inleakage to the secondary containment zone under test.
This is a licensee identified violation.
This is a Severity Level IV Violation (Supplement I.).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) admission or denial of the alleged viola-tion; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
