SUBJECT:

NRC INSPECTION REPORT 50-313/94-07; 50-368/94-07 (NOTICE OF

VIOLATION)

This refers to the inspection conducted by Ms. L. J. Smith and other

inspectors of this office on July 24 through September 3,1994.

The

inspection included a review of activities authorized for your Arkansas

Nuclear One, Units 1 and 2, facility.

At the conclusion of the inspection,

the findings were discussed with you and those members of your staff

identified in the enclosed report.

Areas examined during the inspection are identified in the report.

Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of

activities in progress.

The purpose of the inspection was to determine

I

whether activities authorized by the license were conducted safely and in

accordance with NRC requirements.

Based on the results of this inspection, certain licensed activities appeared

f

to be in violation of NRC requirements as specified in the enclosed Notice of

l

Violation (Notice).

The first violation involves two examples of:

(1) the

failure to lock Unit I feedwater line vent valves which are manual reactor

building isolation valves, and (2) the failure to include the same valves in

the procedure used to check valve positions prior to criticality.

While the

failure of these valves to be correctly positioned during power operations

would quickly be detected, the violation is still of concern because your

staff has incorrectly interpreted that manual reactor building isolation vent

and drain valves on lines which fall undr - the scope of 10 CFR Part 50,

Appendix A, General Design Criteria 57, " Closed System Isolation Valves," do

not have to be locked.

The second violation involving the inadvertent omission of a change to a

maintenance procedure is not being cited because the criteria in

paragraph Vll.B.1 of Appendix C to 10 CFR Part 2 of the NRC's " Rules of

Practice" were satisfied.

The inspection also identified an unre',olved item concerning the low pressure

safety injection motor-operated valves which would not close to a leak tight

configuration in the presence of a design basis check valve leakage.

This

matter is discussed in Section 7 of the enclosed inspection report.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

In your

response, you should document the specific actions taken and any additional

actions ycu plan to prevent recurrence. After reviewing your response to this

Notice, including your proposed corrective actions and the results of future

inspections, the NRC will determine whether further NRC enforcement action is

necessary to ensure compliance with NRC regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of

this letter and its enclosures will be placed in the NP.C Public Document Room.

The responses directed by this letter and the enclosed Notice are not subject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
