l The purpose of this modification is to:
(1) revise the statement of work to reflect a within scope change. (2) increase the contract ceiling amount by 5595.159.00 from 5555.683.00 to 51.150.842.00. (3) increase the obligated dmount by 5164.806.00 from 5300.000 to 5464.806.00. and (4) transfer government equipment accuired from NRC-04-91-054 with SAIC, for the use under i
this contract, in accorcance with your letter request dated August 12. 1994.
Accordingly, the contract is hereby modified as follows:

t STATEMENT OF WORK MODIFICATION NO. 1 RADIATION EXPOSURE MONITORING SYSTEM 1-BACKGROUND i
The purpose of this effort is to assist the DOE Office of Health Physics l
and Industrial Hygiene in: (1) The development of health surveillance support systems: (2) the innovative application of lessons learned from related experiences: and (3) the sharing and transfer of technology currently used by the NRC to support DOE's programs.
2 SCOPE OF WORK The contractor shall develop a prototype system to collect, maintain, and report radiation exposures of DOE employees. DOE contractors, and t
visitors to DOE facilities.
This shall be conducted in accordance with the tasks described below.
Task 1: Radiation Exposure Monitoring System (REMS) Design and Prototype Demonstration 6.
Develop a conceptual design for the system.
b.
Prepare a draft system design report for DOE review and comment.
c.
Develop a prototype demonstration of the system.
d.
Complete a final system design report incorporating DOE comments, NOTE:
The design shall satisfy requirements and objectives identified previously in a system evaluatinn cf the DOE Radiation Exposure Module.
the prototype shall demonstrate the transfer of technologies currently deployed by the NRC integrated into the DOE environment.
Task 2:
System Implementation a.
Test and deploy the system for DOE review.
b.
Prepare a user training plan.
c.
Compile program documentation into a reference manual.
d.
Develop user documentation including user guides and manuals.
Task 3:
Enhancement to the System a.
Implement connectivity capability for data exchange with sites to collect and share information on line.
b.
Migrate to full client server technology, c.
Transport Radiation Exposure Monitoring System (REMS) to other hardware platform, upon request of DOE.

The personnel assigned to this work shall have the following i
qualifications:
a.
Experience in database design and implementation on open systems using Relational Data Base Management System. Fourth Generation Language, and Graphical User Interface technology.
b.
Knowledge of. and experience in. collection, maintenance.
analysis. and reporting of radiation exposure to personnel.
c.
Knowledge of DOE radiation protection directives, mission. and requirements d.
Knowledge and experience of working with radiation exposure and demographic information.
4.
DELIVERABLES TASK 1.
o A draft system design report hall be prov10ed to the NRC and DOE Project Officer for revie'./ and comments by August 31. 1994 o
A prototype demonstration shall be provided to the NRC and DOE September 15. 1994.
