MEXICAN HAT / MONUMENT VALLEY SITE VISIT On May 18, 1994, Daniel Rom and Ted Johnson of the Engineering and Geosciences Branch, and Robert Carlson of my staff, made a construction review visit to the Mexican Hat, Utah, and Monument Valley, Arizona, Uranium Mill Tailings Remedial Action Project sites.
The visits were routine and announced.
On May 18, 1994, the staff observed the construction of the radon barrier at Mexican Hat, and witnessed testing of the rock foundations for the toe of the coll.
Construction testing and inspection records were reviewed at the Remedial Action Contractor office.
Enclosed is the Mexican Hat on-site construction review report for your information.

Areas Reviewed:
The staging area for mixing of sand and bentonite for the radon barrier, the upper surface of the disposal cell, and the toe areas of the cell were observed by T. Johnson and D. Rom.
R. Carlson toured the Monument Valley site with representatives of Morrison-Knudsen (MK) and U.S. Department of Energy (00E).
A copy of the On-Site Construction Review Plan (0CRP) is attached.

Site Review:
Proaress Update (OCRP ltem 1)
The reviewers met DOE and MK personnel on-site at the W office at 9:00 AM.
The staff was briefed on the project status, and orientation was provided to Mr. Carlson.
Messrs. Rom and Johnson then performed a walking tour of the toe of the disposal cell foundation area, and observed rock capability testing operations. A power auger and power excavator were being used by DOE to evaluate rock capability and continuity.
Mr. Rom observed the bentonite mixing operations at the pugmill and radon barrier placement operations on the cell.
Disposal Cell at Mexican Hat (0CRP ltem 1)
The tailings haul to the cell from Monument Valley was complete. Motorized equipment were in the process of placing blended sand-bentonite radon barrier material on the cell. Records indicated that the required minimum 10 percent bentonite was being added to the sand.
The blended cover material a)peared uniform and easy for equipment to place and compact.
The radon aarrier surface was rolled until satisfactory densities were attained.
A small test pad for bedding material had been constructed.
The contractor expressed some concern regarding the ability of the bedding to withstand repeated traffic loadings during the course of placement. Various approaches to controlling such problems, should they occur, were discussed.
Processina Site at Monument Vallev (OCRP ltem II)

Carlson toured the Monument Valley site with DOE and MK personnel.
Remedial action was complete at Monument Valley, and no construction activity was observed.
The perimeter fence around the site was checked and found to be in good condition. The purpose of the fence is to prevent animal grazing while revegetation is occuring. Vegetation was beginning to grow within the fenced-in area at the site.
DOE's mitigation plan for site restoration of wetlands adjacent to the site was accepted by the Corps of Engineers, and the wetlands appeared to be in good condition.

Records Review (OCRP ltem III):
The construction records generated since the time of the last visit were reviewed at Mexican Mat.
The records were found to be complete and up-to-j date, with the minimum test frequency requirements met.
New Proctor test forms were in use. The new forms were computer-generated and included zero-air voids curves.
The new forms correct a potential deficiency, since the l
zero-air -voids and specific gravity information is shown.
Informal interviews were conducted with MK personnel.

At the close of the review the staff met with DOE and MX representatives. It was determined that the field procedures were satisfactory, and that no open issues were identified.
