This letter transmits the findings of the resident safety inspection conducted by Messrs. B. S.

j

Norris, F. P. Bonnett, and R. K. Imrson from June 15 through August 2,1993, at the Peach

'

Bottom Atomic Power Station, Delta, Pennsylvania. The inspectors based these findings on

observation of activities, interviews, and document reviews. The results of the inspection were

discussed with D. Miller and others of your staff.

Our observation of Unit 3 outage activities indicated excellent planning and coordination between

l

working groups. The backfill, on several occasions, of the Unit 2, reactor vessel water level

l

instrument reference leg was well planned and executed. The two Unit 3 start-ups that occurred

this period were well controlled.

Based on the results of this inspection, cenain of your activities appeared to be in violation of

NRC requirements, as specified in the enclosed Notice of Violation (Notice). In three separate

areas the plant staff failed to follow procedures. Specifically, the control room staff did not

effectively control a Unit 2 power restoration resulting in a mispositioned control rod; there were

multiple examples of failure to adhere to radiation worker procedures; and procedural

noncompliances were observed during performance of a Unit 3 hydraulic accumulator level

switch surveillance. The failure to properly implement procedures in three diverse areas is of

concern to the NRC, and could be indicative of less than adequate attention to detail in these

areas.

You are required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response. In your response, you should document the

specific actions taken and any additional actions you plan to prevent recurrence. Additionally,

you are requested to address the specific actions taken in response to each example in the

)

enclosed Notice separately.
