Subject: NRC Region 1 Inspection 50-443/9104 (3/16/91 - 4/22/91)
This refers to the above subject safety inspection. The inspection results are described in the
enclosed report and were discussed with hir. U. Drawbridge of your staff.
,
Overall, this inspection found that the plant was operated safely. IIxcellence was noted in
operator response to events and station management's safety perspective in deciding to fully
investigate the cause of the feedwater check valve problem.
Certain occurrences during the inspection period indicate inadequacies in workers' attention-
to-detail For example, a Safeguards Information container was left unlocked even though
corrective actions for a previously cited violation were being impler..ented. Othcr examples,
including a self-identified out-of-position locked val,e indicate a possible need for additional
management attention to prevent similar errors. As discussed in Paragraph 2.4, the
acceptability of your program for controlling valve positions is considered unresolved.
Although no written response to this letter is required, these items are noted for your
attention.
