TVA letter to NRC dated January 25, 1991, "Sequoyah Nuclear Plant (SQN) - Units 1 and 2 - Look-Back Review of Cable issues ' TAC No. 77129/77130)"
By the referenced letter, TVA supplied NRC with the TVA report summarizing.the look-back review of cable issues performed by the Nuclear Quality Audit and Evaluation Organization.
Subsequent to issuance of this report, additional information was obtained that revealed minor discrepancies in the report. TVA is providing these corrections to NRC to supplement the docketed report in the referenced letter. The changes are being made to ensure accuracy, but are not considered to alter the report conclusions'in any substantive manner. The enclosure provides the corrections to the look-back review report.

Supplement to hook-Back Review of Cable lorran This enclosure supplemento TVA's report summarizing tho io, onck review of cable issues performed by the Nuclear Quality Audit and Evaluation (NQA&E)
Organization, The original look-back review report indicated that in Nuclear Engineering procedure (NEP)-3.1, Revision 0, " Calculations," the manager responsible for initiating electrical calculations was the lead electrical engineer for project calculations and the accistant branch chief for branch calculations.
NEp-3.1, Revision 0, actually assigned responsibility to the lead cicctrical engineer or group head for electrical calculations.
(Revision 1 of NEp-3.1, in general, substituted " Assistant Branch Chief" for group head whero it appeared in Revision 0.)
Subsequent to the referenced report, NQA&E was made aware of another procedure Procedure Mothod 81-27, Revision 2. " Delegation of Signaturo Authority and Conduct of Specialipto Reviews." This procedure delegated the responsibility of signature authority to the anoistant branch chief for branch calculations and to the lead engineer for project calculat. ions.
It is apparent from the interviews of the Sequoyah Nucicar plant lead j
electrical enginecro and the former asciutant branch chief, documented in paragraph V.D. of the referenced report, that there was not a clear understanding as to whether the subject calculations were " project initiated" or " branch initiated." Accordingly, based on those interviews and review of the above procedural controls, the conclusions reached in paragraph V.A.-7, page 7 of the referenced report, remair unchanged.
One additional minor report correction in that in the referenced report, all references to calculation SQN-CSTF-008 should read EEB-CSTF-0008,
