ATTACHMENT Hotice of Violation:

10 CFR 50, Appendix B, Criterion XVI requi"es measures to assure that conditions adverse to quality shall be promptly identified and corrected.

Contrary to the above, measures to assure prompt correction of conditions adverse to quality were not taken on March 11, 199:, when operators identified a condition adverse to quality in Procedure 3.1.4, " Emergency Diesel Generator Surveillance Testing," Revision 33.

In this case, the procedure improperly indicated that load was to be applied to the Emergency Diesel-Generator, while keeping the reactive loading at double the resistive loading, and action was not initiated to correct this condition adverse to quality.

Maine Yankee Response:

In January of 1991, the adequacy of Maine Yankee's monthly emergency diesel generator testing progran, was questioned. A procedure change report (PCR) was written modifying the diesel generator surveillance procedure (Procedure 3.1.4) to require reactive load be applied in addition to resistive load.

The PCR specified the ninimum reactive load to be applied and provided guidelines limiting the reactive / resistive load ratio to prevent overloading the diesel generators. As oescribed in the inspection report there was some confusion on l

the restrictions of the PCR, and control room personnel appropriately discussed and resolved the matter, and governed their actions properly. Immediate action to improve the wording of the PCR, however, was not taken.

Immediate Corrective Actions and Results Achieved:

As described in the inspection report, upon recognition that the procedure change report should be clarified, Maine Yankee initiated a review of the surveillance procedure guidance.

A clarification of the diesel surveillance procedure was subsequently developed and a revision to the procedure was drafted.

Corrective Steps Taken to Avoid Further Violation:

Procedure 3.1.4 was revised on March 22, 1991 incorporating clarifications and human factors improvements to the monthly surveillance procedure. This revised procedure was reviewed by PORC and issued prior to the next scheduled emergency diesel generator surveillance.

A ctgy of this citation and Maine Yankee's response has been issued for Operations Department crew document review.

Full Compliance Date:

Full compliance was achieved on March 26, 1991 when Procedure 3.1.4, Revision 34, was successfully used to conduct surveillance testing.
