!.OfRICELLO fUCLEAR GENERAT lfG PLAfD Docket No. 5-263 DPR-22 Head Spray isolation Valve Failing to Close A recent occurrence at the Monticello Nuclear Generating Plant is hereby reported to you in accordance with Section 6.6,C.1 of the Technical Specifi-cations.
On May 13,1972 the RHR System was put in the shutdown moling mode of operation with the head spray systeg in service. When the reactor moderator temperature attempted to close the outboard aead spray isolation valve (LO 2026)perator reached approximately 126 f the RHR pumps were shutoff.

During normal operation of the plant these valves are closed.
The failure was found to be due to the actuating arm of the 42 M auxiliary contactor in the valves cotor control center (D31308). This arm was readjusted and the valve was cycled to insure that it operated properly. The closing feature of the valve was inoperable for approximately sixteen hours.
A Significant Operating Event report has been prepared for this occurrence and is avcilable at the Plant for review by Region lil Compliance Inspector.
